Qualifications
  • High School diploma or equivalent. Bachelor’s degree a plus.
  • At least two (2) years of experience working in a healthcare or clinical setting in support of persons served.
  • Must possess a valid driver’s license in the State of Michigan and proof of insurance.
  • Ability to work closely and communicate effectively with persons with mental and/or physical limitations.
Duties and Responsibilities
  • Establish trusting relationships with persons served and collaborate effectively with clinical care teams.
  • Provide ongoing interviewing, goal setting and follow‑up for persons served.
  • Effectively communicate Easterseals MORC’s mission in establishing relationships with medical staff and persons served.
  • Act as an advocate and liaison between persons served and community service agencies.
  • Assist persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services.
  • Connect persons served with transportation resources when appropriate.
  • Assist with the completion of required information such as enrollment, eligibility determination and overall follow‑up; assist in completing applications and registration forms when needed to expedite care.
