- High School diploma or equivalent. Bachelor’s degree a plus.
- At least two (2) years of experience working in a healthcare or clinical setting in support of persons served.
- Must possess a valid driver’s license in the State of Michigan and proof of insurance.
- Ability to work closely and communicate effectively with persons with mental and/or physical limitations.
- Establish trusting relationships with persons served and collaborate effectively with clinical care teams.
- Provide ongoing interviewing, goal setting and follow‑up for persons served.
- Effectively communicate Easterseals MORC’s mission in establishing relationships with medical staff and persons served.
- Act as an advocate and liaison between persons served and community service agencies.
- Assist persons served in overcoming barriers to obtaining needed medical care and/or behavioral health/social services.
- Connect persons served with transportation resources when appropriate.
- Assist with the completion of required information such as enrollment, eligibility determination and overall follow‑up; assist in completing applications and registration forms when needed to expedite care.
