We are seeking an
enthusiastic and hardworking Social Worker
to joinour
Transition Team
, who cover the
Wakefield District
.
You will work closely with key partners in Children’s Services to carry out
person‑centred assessments
with young people aged
16–25
who are transitioning from Children’s services into
Adult Social Care
.
Using a strengths‑based approach to assessment and support planning, you will:
- Promote independence and self‑determination
- Focus on positive change and prevention of harm
- Support young people to achieve their personal outcomes
- Undertake
holistic assessments early in the transition process
to ensure a smooth and successful transfer into Adult Social Care
The Transition Team consists of4.5 FTE Social Workers
. You will be joining a
motivated, supportive, and forward‑thinking team
where supervision, development, and collaboration are priorities.
- Monday to Thursday:8:30am – 5:00pm
- Friday:8:30am – 4:30pm
- Flexible working arrangements
are available and can be discussed with the line manager
As a Social Worker, you will:
- Be passionate about making a difference and open to change
- Demonstrate the ability to work under pressure
- Be well‑organised and able to prioritise and manage risk within a complex caseload
- Have experience and a strong commitment to strengths‑based practice
- Be able to undertake standard social care duties, including:
- Chairing meetings
- Providing support and advice to junior staff
- Producing high‑quality, person‑led assessments and reports (including court reports)
A
full driving licence
is essential, as the role covers the whole Wakefield District and some young people may be placed out of area.

Assessed and Supported Year in Employment (ASYE)programme. Wakefield is committed to supporting NQSWs, offering:
- A dedicated ASYE assessor
- Regular 1:1 supervision sessions
- Training and team support
Your first year will be a probationary period, during which we will work closely with you to support your development and progression in your Social Work career.
Essential RequirementsApplicants must:
- Hold a
Degree in Social Work
or equivalent - Be
registered with Social Work England - Have relevant
post‑qualifying training and experience - Hold a
full driving licence
