We are seeking an enthusiastic and compassionate Practice Nurse to join our friendly, forward-thinking primary care team. This is an exciting opportunity to play a key role in delivering high-quality, patient-centred care within a supportive and collaborative practice environment. You will work closely with GPs and the wider multidisciplinary team to make a genuine difference to the health and wellbeing of our local community.
The main purpose of the Practice Nurse post is to deliver high-quality,patient-centred nursing care within a primary care setting, supporting thehealth and wellbeing of the practice population.
The Practice Nurse contributes to improving patient outcomes, maintaining clinical standards, and ensuring safe, effective, and compassionate care inline with professional and regulatory guidelines.
Main duties of the jobWe are seeking a skilled and motivated Practice Nurse to deliver high-quality, patient-centred nursing care within our busy primary care setting. The successful candidate will play a key role in supporting the health and wellbeing of our practice population by providing evidence-based clinical care and promoting positive health outcomes.
This varied and rewarding role includes chronic disease management (including diabetes), minor illness assessment and treatment, health promotion, screening, immunisations, and treatment room services. Working collaboratively with GPs and the wider multidisciplinary team, you will contribute to maintaining excellent clinical standards and delivering safe, effective, and compassionate care in line with professional and regulatory guidance.
