Overview
The Mid Yorkshire Teaching NHS Trust is looking for a Band 7 Stroke Assessment Nurse to join our Stroke Hub based in the hyper acute stroke unit. We provide life saving treatment such as thrombolysis and anticoagulation reversal. The stroke Hub is a fast paced rewarding environment accepting patients from Wakefield Dewsbury and Barnsley.
As a stroke assessment nurse you would be one of the first points of contact for patients with new neurological symptoms of stroke. Responsible for the rapid identification, assessment, and management of patients with suspected acute stroke or TIA, ensuring timely access to hyper-acute stroke care in line with NICE guidelines and local stroke pathways.
Main dutiesRole Specific Responsibilities for CNS in Stroke Assessment
- Hold the on-call phone for all referrals to the Stroke Team from inside and outside the trust.
- Formulate a provisional diagnosis by undertaking a comprehensive advanced patient assessment based on history taking and clinical examination, using stroke specific assessment tools and implementing a management/treatment plan.
- Refer to other members of the multidisciplinary team and other specialities as appropriate.
- Work autonomously taking responsibility for own decision making, referring to the stroke consultant on call when further advice and clinical input is needed or when care and treatment goes beyond normal service parameters or scope of professional practice.
- Work within a frequently challenging clinical environment with high levels of physical and mental effort.
- Have knowledge of pathophysiology, anatomy, and physiology to assess, plan, implement and evaluate treatment of patients presenting with stroke symptoms and TIA including mimics.
- Accurately request pathology and radiological investigations and make clinical decisions based upon the results.
- Facilitate the transfer of stroke patients to the stroke ward, repatriate, and discharge as appropriate.
- Competently and confidently care for stroke / TIA patients utilising expert skills and knowledge in a variety of clinical settings as part of the multi-disciplinary team.
