Position: Senior Social Worker and AMHP (Mental Health Transitions age 16 – 25) - 244694

Are you passionate about supporting young people at a critical stage in their lives? We are looking for a Mental Health Transitions Senior Social Worker to join our Adult Mental Health Social Work Teams. This is a unique opportunity to shape how young people aged 16+ move from children’s to adult mental health services, ensuring continuity, stability, and person‑centred care.

About the Role
  • Work across mental health teams to support young people 16+ as they transition into adulthood.
  • Provide the consistent and specialist support young people need as they navigate moving to adult mental health services – so this is the best experience possible.
  • Collaborate with CAMHS, Children’s Services, SWYPT, and ICB colleagues to create seamless pathways.
  • Champion trauma‑informed, creative, and recovery‑focused approaches.
  • Empower young people to take ownership of decisions about their future.
About You
  • Motivated and experienced Senior Social Worker with strong knowledge of statutory social work duties, focused on mental health and/or transitions and a readiness to learn and develop specialist expertise.
  • Demonstrable expertise in strengths based, person‑centred planning and supporting people through change.
  • Demonstrates sound understanding of Children’s Legislation, the Care Act 2014, Mental Health Act 1983 (MHA), Mental Capacity Act 2005 (MCA), and Human Rights legislation.
  • Values collaboration, innovation, and positive risk‑taking.
  • Committed to promoting independence, rights, and sustainable outcomes.
