We are looking to recruit an experienced and conscientious part time Care Navigator to work within a friendly team of 6 CARE NAVIGATORS all with a varied level of experience of working in a doctors reception.

The hours will be a total of 22.5 hours across 5 days a week working from 14:00 - 18:30pm. Some flexibility will be required to cover the morning session for sickness and holidays

Full training will be given but Medical Administration and Receptionist experience in a GP Practice would be advantageous. Experience of the Systm One clinical system is desirable but not essential as training can be given.

The purpose of this role is to work as part of a team tomeet its agreed aims and objectives within an efficient, safe and effective working environment, ensuring that services are meeting the needs of itsregistered patients and all regulatory requirements

We are a small urban Practice of about 2700 PTs and have transformed the level of care our patients receive in the last 12 months and we have every intention of continuing to improve this year on year. We are CQC rated GOOD and have hit all of our targets both locally and nationally

Job responsibilities
  • Receive, assist and direct patients in accessing the appropriate services in a courteous, efficient and effective way
  • Undertake a variety of administrative duties to assist in the smooth running of the Practice
  • Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice
  • Issuing patient on-line access passwords
  • Deal with all general enquiries, explain Practice procedures to patients
  • Receive and make telephone calls as required. Divert calls and take messages, ensuring accuracy of detail and prompt appropriate delivery.
  • Make appointments and telephone consultations ensuring callers are directed to the appropriate healthcare professionals
  • Enter requests for home visits onto Systm One, ensuring careful recording of all relevant details
  • Communicate test results appropriately being aware of confidentiality at all times
  • Handle blood and urine samples in line with infection control procedures
  • Diptest urine samples and communicate results to the clinician
  • Process repeat prescription requests and ensure that they are ready for collection by the patient within 24 hours
  • Advise patients of relevant charges for private (non NHS) services, accept payment and issue receipts for same.
  • Enter patient information on to the computer as required.
  • Ensure the kitchen is kept clean and tidy and the dishes are cleaned and put away as required.
  • Keep the reception area, notice-boards and leaflet dispensers tidy and free from obstructions and clutter
  • Providing clerical assistance to Practice staff as required, including scanning, coding, filing and photocopying
  • Ensure Practice is closed down safely at night to include closing of window and shutters
  • Undertake any other additional duties appropriate to the post as requested by the Management Team
Person Specification Experience

Qualifications
  • Good Standard of Education GCSE Grade C in English and Maths
  • Previous experience as a Care Navigator/GP Receptionist/administrator
