Lead Community Health Worker (Lead CHW)

The Lead Community Health Worker contributes to meeting comprehensive patient needs under the Primary Care Medical Home (PCMH) model. Core duties include patient recruitment, education, navigation, social support, medical follow‑up, chronic disease management, and care coordination.

• Recruit and educate patients for outreach across Maui Island (excluding Hana).
• Coordinate and integrate care with patients, partners, families, and caregivers.
• Identify resources, set goals, assist with self‑monitoring, and deliver culturally and age‑appropriate education in multiple languages.
• Manage group health education classes, marketing of community events, and enrollment capacity.
• Assist with appointment scheduling, psychosocial support, eligibility referrals, transport coordination, and Patient Assistance Program.
• Prepare reporting data, participate in grant reporting, and maintain data for grants.
• Supervise Community Health Workers: schedule, approve timecards, recruit, train, manage, and provide feedback.

• High School Diploma or equivalent.
• Experience:
  
  1‑2 years in community‑based, multicultural settings.
• Fluency in English (other languages desirable).
• Knowledge of medical terminology and basic computer skills.
• Strong communication, trauma‑informed care, emotional support skills.
• Valid driver’s license, insured vehicle, TB clearance within 12 months, BLS CPR certificate.

The Lead CHW makes independent routine decisions within scope and refers non‑routine decisions to manager or director.
