Location: Nagel Assisted Living – Waconia, MN

Schedule: Overnight Shift | Every Other Weekend + Rotating On-Call

What You’ll Do

• Provide Personalized Care: Plan, deliver, and monitor nursing care tailored to each resident’s needs.
• Conduct Assessments: Perform comprehensive health evaluations and accurately document findings.
• Manage Medications: Administer medications and monitor chronic conditions to optimize residents’ health.
• Coordinate Care Plans: Collaborate with staff, families, and external providers to create individualized care plans.
• Foster Communication: Keep families informed of changes in health or care plans to build trust and transparency.
• Respond to Emergencies: Act promptly to address urgent resident needs, ensuring appropriate follow-up actions.

What You Bring to the Role

• Licensed Professional: A valid Licensed Practical Nurse (LPN) license in the state of Minnesota.
• Geriatric Care
  
  Experience:
  
  Previous experience in senior care is highly desirable.
• Independent Worker: Ability to work autonomously while maintaining high care standards.
• Organized and Detail-Oriented: Strong planning and communication skills with attention to detail.
