RN Case Manager – Waco & Surrounding Counties

Alamo Hospice is hiring an RN Case Manager to support patients and families facing end‑of‑life journeys. We offer comprehensive benefits including generous PTO, 401(k) match, and competitive salaries.

  • Shift: Monday‑Friday 8a‑5p
  • On Call: 1‑2 weeknights, occasional weekend if needed

What You’ll Do
  • Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain and assess data from patient physical, psychological, social, and spiritual factors to impact needs and initiate interventions with interdisciplinary team members
  • Initiate communication with attending physician, interdisciplinary team, and other agencies for optimal care coordination
  • Provide regular visits to reassess patient needs and update the plan of care
  • Perform clinical pronouncement of death and provide support to the family
  • Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely
  • Supervise and document care delivery by Home Health Aides as required by federal, state, and company guidelines
Qualifications
  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Current CPR and First Aid certification
  • Knowledgeable of quality assessment and assurance procedures
  • Valid driver’s license and proof of insurance
