Job specification
- Coordinate and manage care for patients, ensuring that appropriate services and resources are provided.
- Collaborate with physicians, social workers, and other healthcare professionals to create individualized care plans.
- Assess patients' needs and identify available resources, ensuring the effective delivery of services.
- Monitor patient progress and adjust care plans as necessary to improve outcomes.
- Educate patients and families on care plans, treatment options, and healthcare resources.
- A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing (MSN) or related field is preferred.
- Current Registered Nurse (RN) license in the state of Texas.
- Minimum of 3 years of clinical nursing experience, with at least 2 years in a case management or care coordination role.
- Case Management Certification (CCM) preferred.
- Basic Life Support (BLS) certification is required.
