We are seeking a compassionate and skilled Post‑Acute Advanced Practice Provider (APP) to join our care team. In this role, you will support patients and families as they transition from hospital to post‑acute settings, managing recovery and complex health needs with expertise and empathy.

The APP will provide comprehensive medical care, symptom management, and emotional support, working to enhance comfort, promote healing, and improve quality of life for every patient throughout their recovery journey.

Starting salary: $ annually. The actual compensation package could vary based on factors such as, but not limited to, the applicant’s experience, internal equity, and alignment with market data.
