• LSW or CSW license required; MSW preferred.
• Previous experience in sub‑acute or long‑term care settings.
• Identify residents’ psychosocial, mental, and emotional needs.
• Complete social history and psychosocial assessment for residents.
• Collaborate with the interdisciplinary team to promote and protect resident rights.
• Prevent and address resident abuse as mandated by law and professional licensure.
• Perform additional duties as assigned by the Director of Social Services and Administrator.
• Work independently or as part of a group.
• Computer proficiency, including experience with Point Click Care electronic medical records.
• Build and maintain rapport with residents and peers.

• Full‑time, day shift (including 1 Manager on Duty weekend every 5 weeks).
