Summary
We are seeking a dedicated and compassionate full-time Hospice Social Worker to join our team. In this role, you will provide psychosocial support to patients and their families, helping them navigate emotional, social, and financial challenges associated with end-of-life care in the Cape May, Atlantic, and Cumberland County, NJ region. Your expertise will contribute to the development of comprehensive care plans and support our interdisciplinary team in delivering exceptional hospice care.
Responsibilities
- Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program. This includes emotional, social, financial and environmental resources and identifying appropriate psychosocial problems, interventions and goals for the patient’s plan of care.
- Participate in IDG meetings and the development of the patient’s plan of care.
- Observe, record and report changes in the patient's emotional and social factors affected by illness and need for care and response to treatment.
- Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.
- Evaluate the patient and family’s response to, and effectiveness of, the medical social work intervention.
- Confirm, on a weekly basis, the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits.
- Participate in IDG and QAPI meetings and activities.
- If the patient/family declines Social Work services, the Social Worker continues to offer support to the IDG in care of the patient and to monitor evolving needs.
- Commit to self-development through formal education, workshops and conferences, professional meetings, and/or independent study.
- Assess the patient/family’s ability to cope with the patient’s dying.
- Act as consultant to health team members, helping them understand social, emotional and environmental factors related to health problems.
- Help patients utilize family and community resources.
Qualifications
- Licensed by the NJ State Board of Social Work Examiners with a master’s degree in social work from a CSWE-accredited program.
- Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- At least one year of post-master’s social work experience in a health care setting.
- Hospice experience preferred.
- Ability to reliably handle and maintain confidential information.
- Must be a licensed driver with an insured automobile in good working order.
- Strong verbal and written communication skills.
Compensation
Pay range from $64,750
