Responsibilities
  • Under the supervision of a professional nurse, provide direct care to residents in alignment with Brookdale’s philosophy and standards of home health care.
  • Assist residents with activities of daily living (bathing, dressing, grooming, toileting, transferring, and getting to meals/activities) according to the resident care/service plan.
  • Encourage residents to participate in their own care as much as possible while maintaining dignity and respect.
  • Document the delivery of quality care in the resident’s clinical record and follow infection control and safety policies.
  • Ensure care is accurately, timely, and appropriately documented; maintain confidentiality at all times.
  • Maintain a clean, safe, and orderly environment; perform general housekeeping duties as assigned.
  • Respond promptly to resident requests for assistance, including emergencies.
  • Communicate effectively with residents, families, and co-workers to promote a positive working relationship.
  • Identify changes in resident status or needs and communicate promptly to the nurse.
  • Participate in educational programs as required.
Qualifications
Education and Experience
  • High school diploma or GED; and minimum of one to three months related experience and/or training; or equivalent combination of education and experience.
  • Previous experience in a health care setting is preferred but not required.
  • Must provide certificate showing completion of 75-hour home health aide curriculum.
Certifications, Licenses, And Other Special Requirements
  • In accordance with state law, may need to possess current state certifications and follow regulations to maintain current certification.
Management/Decision Making
  • Makes standard and routine decisions based on detailed guidelines with limited independent judgment. Problems are generally solved with clear guidelines or by reporting to a supervisor.
Knowledge And Skills
  • Basic reading, writing, and communication skills; ability to follow written or verbal instructions.
  • Basic technical skills to complete a few repetitive and well-defined duties.
  • Ability to read simple instructions and memos; write simple correspondence; communicate effectively in English with residents, families, and associates.
  • Good judgment in emergency situations and the ability to seek assistance when needed.
Physical Demands and Working Conditions
  • Standing, walking, use of hands and fingers, reaching with hands and arms, climbing or balancing, stooping/kneeling/crouching/crawling.
  • Ability to lift up to 50 pounds; vision and ability to interact with coworkers, residents, or vendors.
  • Occasional weekend, evening, or night work as needed for shift coverage.
  • Possible exposure to communicable diseases, blood-borne pathogens, and various hazards; exposure to latex and chemicals.
