** San Diego area - Chula Vista specifically.
** We are currently searching for
** a experienced Nurse Practitioner or Physician Assistant
** to engage in Medicare Wellness Visits at our contracted partner clinics and the occasional home visit. This role requires fluency in Spanish.
** This is a full-time position of 5 days per week, 8-hour days (8am - 5pm / 40 hours per week)

* Responsibilities include:

*** Perform Wellness Visits in partner primary care clinics and facilities in the Chula Vista community.
* Drive to designated facility locations in your assigned geographic area.
* Review patient medical and family history.
* Review of current providers and prescriptions.
* Perform height, weight, blood pressure, and other routine measurements.
* Deliver personalized health advice.
* Review risk factors and potential treatment options.
* Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
* Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
* Provide referrals to appropriate medical resources.
* Utilize our EMR system to document patient visit/interaction accurately and timely.
* Knowledge of HCC coding.
* Other duties as assigned.

** Qualifications and experience:
*** Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
* Current, unrestricted NP or PA license to practice in California, including DEA license.
* Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient setting.
* Minimum of 1 year of previous experience utilizing an EMR system.
* Spanish speaking mandatory
* Good working knowledge of HCC coding.
* Strong computer skills including MS Office and Outlook is strongly desired.
* Great verbal and written communication skills
* Current drivers’ license, auto insurance and reliable transportation
* Compassionate and caring attitude with a desire to live our mission, vision and values.
* Pay Rate:  $140,000 - $150,000 annual base salary plus bonus
