Early Case Manager; Part-Time

This position is uniquely designed for individuals who are a parent, grandparent, or sibling of a child with special needs. The Special Needs Case Manager plays a vital role in supporting families by providing case management services, resource navigation, and emotional support tailored to each family's circumstances.

• Provide individualized case management and linkage services to families of children diagnosed with special needs.
• Collaborate with families to assess needs, identify priorities, and establish personalized goals.
• Regularly review and update goals in alignment with the evolving needs of each family.
• Offer support and guidance through both home and office visits as part of the case management process.
• Assist families in navigating local, state, and federal resources and services.
• Deliver emotional support and help families develop empowerment skills for long-term self-sufficiency.
• Maintain accurate documentation of all case activities and ensure compliance with organizational and regulatory standards.

• Must be a parent, grandparent, or sibling of a child with special needs.
• Demonstrated understanding of the challenges faced by families of children with special needs.
• Strong interpersonal and communication skills.
• Ability to work independently and collaboratively with families and service providers.
• Reliable transportation for home visits.
• Prior experience in case management, social services, or related field is preferred but not required.

• Ability to travel locally for home visits.
• Flexible work hours may be required based on family needs.
• Must be comfortable providing services in various settings (e.g., homes, offices, community spaces).

• BA or 2 years in case management (Preferred)
• Required:
  
  Be the parent, grandparent, sibling or other relative (who has helped raise the sibling) of a child diagnosed with a special need
• Must have reliable transportation
• Valid CA Driver’s License /Ability to pass a Criminal Background Check

$18.00 - $20.00 per hour
