• Patient Care:
  Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs; provides a complete physical assessment and history of current and previous illness(es); utilizes the nursing process per the state Nurse Practice Act; writes and initiates plan of care and regularly reevaluates patient and family/caregiver needs; initiates plan of care revisions as needed; uses health assessment data to determine nursing diagnosis;
  
  develops a care plan with goals and involves patient and family; initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed; counsels patient and family; provides health care instructions; assists with activities of daily living and promotes self-sufficiency; acts as Case Manager when assigned.
• Communication:
  Documents clinical notes and tracks pain/symptom management; communicates with physician and coordinates care with the interdisciplinary group; provides patient and family education and health counseling; maintains a safe environment and ensures continuity of care; attends interdisciplinary group meetings.
• Additional Duties:
  Participates in on-call duties; ensures equipment and services are available; supervises ancillary personnel as required; pursues ongoing professional development; fulfills case assignments; participates in quality assessment and performance improvement activities.

• Graduate of a National League of Nursing accredited school of nursing.
• Current license in California and current CPR certification.
• RN with either (a) at least one year of professional nursing experience within the last three years, or (b) a baccalaureate degree in nursing from an NLN-accredited program and a current RN license; preferred: minimum of two years' experience, at least one year in public health, home care, or hospice nursing.
• Must be a licensed driver with a valid insured automobile in good working order.
• Self-directed, able to work with minimal supervision, and strong communication and nursing skills; ability to perform required nursing procedures and handle activities of daily living for patients.
