* * 8750 Quality & Patient Safety The Quality Improvement Manager, in collaboration with medical staff and leadership teams, identifies opportunities for process improvement and facilitates continuous quality improvement efforts by developing systems necessary to further assess, analyze and trend data in a manner consistent with industry standards of practice and to support a data-driven quality improvement program throughout the Kaweah Health.

QUALIFICATIONS License /Certification

Required:

CA RN license    Preferred:
Certified Professional in Healthcare Quality (CPHQ)
Education

Required:

BSN Lean Six Sigma Black Belt Certification within two and a half years of hire/transfer    Preferred:
Master’s degree in Nursing, Public Health, or Business Administration Experience

Required:

Two years of quality improvement experience.

Experience with quality initiatives and methodologies, such as: FOCUS PDCA, RCA, FMEA, CQI method and tools, group facilitation, working knowledge of Statistical Process Control methodology, Lean, and Six Sigma principles.
Preferred:
Management experience

Knowledge/Skills/Abilities  Excellent oral and written communication skills.  A wide degree of creativity and latitude is expected.  Relies on experience and judgment to plan and accomplish goals.
JOB RESPONSIBILITIES  Essential    Drives improvements in patient safety and quality, and operational efficiency by identifying, planning and facilitating continuous improvement activities together with staff leadership and physicians.
Creates an avenue for transparency with leadership, physicians and staff in order to engage them in improvement efforts.
Manages collection and analysis of clinical outcomes data in order to develop clinical process improvement initiatives.
Manages staff who gather data on performance metrics and facilitates the review of program effectiveness. Manages Core Measure Abstraction Team, Sepsis Coordinator, and Rapid Response RN Clinical Analyst, holding each team member accountable for performance. Performs personnel functions including: hiring, training, mentoring, annual performance reviews, disciplinary actions/counseling.
Evaluates patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards. Performs analysis of data to identify potential opportunities for improvement in meeting core measures and public reportable metrics. Identifies and reports trends identified through monitoring the Quality Improvement and Patient Safety plan and develops action plans to improve. In collaboration with hospital and physician leadership, selects and implements solutions to develop appropriate monitor and control plans to ensure implementation and hard wiring of improvement/change.
Develops and implements an ongoing surveillance program to monitor new measures, standards or guidelines specifically for core measures and public reportable data.
