Summary:
We are seeking a caring and compassionate Licensed Practical Nurse (LPN) who will provide excellent direct care to our residents on a daily basis. The incumbent will perform assigned nursing procedures for the comfort and well‑being of residents such as assist in admission of new residents; assist with activities of daily living when needed; administer specified medications; supervise non‑licensed nursing staff and participate in the evaluation of non‑licensed staff.

Duties and Responsibilities
Demonstrates excellent decision‑making skills and can understand the impact of those decisions.*Sets and manages priorities.*Accepts calls from staff during nights and weekend.*Meets with oncoming and departing licensed practical nurse to discuss resident's condition and any occurrences from the shift.*Responsible for making sure communications are clear, accurate and timely.*Counts controlled substances with off going nurse, assumes responsibility for assuring accuracy of count before accepting keys.*Checks, orders, and administers prescribed medication with 100% accuracy.*Responsible for always ensuring the correct dosage by never pre‑pouring medications.*Demonstrates knowledge of important medication interactions and side effects.*Ensures medication cart is locked at all times.*Obtains pertinent information and completes all required forms as established by policy and procedure and ongoing thereafter, upon resident admission.*Responsible for completing and maintaining the ECC and LNS logs, service plan, progress notes, monthly summaries and follows healthcare providers ordersAssists in completing evaluations when needed for residents.*Provides hands on care on an as needed basis to meet the needs of the residents.*Works with the ADORS in reviewing and investigates all incidents reports and recommends appropriate action and follow up to the DORS/executive director.*Understands and adheres to the ALF regulations pertaining to assisted living health care laws.*Maintains a safe and secure working environment free of objects/situations that could cause harm and/or falls to residents, families, visitors and associates.*Responsible for always practicing good body mechanics and safe working habits.*Knows, understands, and follows established policy and procedures in emergency situations.*Takes time to give associates in nursing daily feedback on his/hers performance.*Participates in the associates performance evaluations when request by the DORS.*Responsible for ensuring associates participate in activities provided to residents as described in the activities calendar.*Responsible for always responding promptly and courteously to resident's request for assistance including phone calls and request from family and visitors.*Ensures there is a safe, clean and orderly environment for all residents.*Responsible for always ensuring and supporting a dignified and caring atmosphere for residents, families, visitors and associates.*Responsible for always wearing clean and appropriate dress per established policy.*Understands and complies with HIPAA regulations.*Responsible for being considered essential personnel especially during emergencies.*Takes the initiative in work, seeks out additional information and offers solutions/suggestions.Performs all other reasonable duties as assigned or as requested.Responsible for willingly taking on special projects or assignments outside of regular duties.Supervisory Responsibilities
