NP​/PA at Lively Medical Center

Lively Medical Center is seeking a Family Nurse Practitioner (FNP) to join our established outpatient primary care team. This is a full-time, outpatient-only position offering a consistent schedule and strong clinical support.

The ideal candidate is committed to delivering comprehensive primary care across the lifespan and values working within a collaborative, team-based model.

• Outpatient-only practice

• Schedule:
  
  7:00 AM - 5:00 PM, 4 days per week.

• Patient Volume: 18-22 patients per day

• Call:
  Rotating call schedule shared among 10+ providers

• Supported by dedicated clinical staff (CMA/LPN) for rooming and patient flow

• Provide comprehensive primary care services including:

  • Routine wellness visits

  • Medicare Annual Wellness Visits

  • Acute care visits

  • Hospital follow-ups

  • Preoperative examinations

  • Chronic disease management

• Current certification as a Family Nurse Practitioner

• Active and unrestricted state licensure

• DEA certification (or eligible)

• Commitment to high-quality, patient-centered care

• Strong communication and teamwork skills
