About the Role

We’re seeking a compassionate Registered Nurse (RN) to join our hospice team—available full-time, part-time, or PRN. As an RN Case Manager, you’ll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You’ll collaborate with an interdisciplinary team to ensure every patient is cared for—body, mind, and spirit.

How You’ll Make a Difference

  • Assess patient and family needs and develop personalized care plans.
  • Deliver skilled, compassionate nursing care and symptom management.
  • Administer medications and coordinate care across the healthcare team.
  • Educate and support families to promote comfort and peace of mind.
  • Lead and guide hospice aides to ensure consistent, compassionate care.
  • Maintain accurate documentation and participate in ongoing education.

What You’ll Bring

  • Current RN license in Virginia (multi-state or single-state).
  • CPR certification and at least two years of nursing experience (hospice or home health preferred).
  • Strong clinical judgment, communication, and organizational skills.
  • Reliable transportation, valid driver’s license, and insurance.
  • A self-directed, compassionate spirit ready to serve your community.
