Primary Care APP 's and Family

Augusta Health is launching a dedicated Team Member Clinic to provide exceptional care to the people of August Health and their families. Were seeking an experienced Nurse Practitioner or Physician Assistant to join this collaborative, mission-driven setting. In this role, youll provide comprehensive outpatient care including wellness visits, chronic disease management, and same-day acute carewhile working closely with physicians, nurses, health coaches, and administrative staff.

Youll engage in care coordination, patient education, and team huddles, all supported by a streamlined clinical and administrative infrastructure to ensure high-quality, efficient care delivery.

Practice Details

• Schedule: Monday-Friday, approx. 9 AM-5:30 PM

• Patients: 14-16 per day (mix of scheduled and acute visits)

• Call: Shared with the Primary Care service line; planned annually
