• Flexible Scheduling: Work when it suits you! Assessments are conducted Sunday to Saturday from 8 AM to 7 PM.

As a Matrix provider, you’ll conduct in-depth assessments that go beyond the typical office visit. Your responsibilities will include:

• Conducting comprehensive assessments of adult and geriatric patients, including medical history and current health status.
• Administering and ordering necessary tests.
• Collaborating with healthcare teams, including physicians and social workers, to enhance patient care.
• Providing services in the comfort of patients’ homes or skilled nursing facilities.

To thrive in this role, you should have:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP or PA licensure in your state, including prescription authority.
• Board certification in relevant specialties.
• Current BLS, ACLS, or CPR certification.
• At least 1 year of experience as a Nurse Practitioner or Physician Assistant (new grads are encouraged to apply!).
• Strong communication skills and proficiency with medical record software.
