As a valued member of our team, you will:

• Conduct comprehensive health assessments, including medical history, diagnosis, and treatment plans.
• Administer and order necessary lab tests.
• Collaborate with healthcare professionals, including physicians and case managers.
• Provide care in the comfort of patients’ homes or skilled nursing facilities.

To thrive in this role, you should have:

• A Master’s Degree or equivalent experience with NP or PA licensure.
• Current NP or PA licensure in your state, including prescription authority.
• Board certification in relevant specialties.
• Strong communication skills and a compassionate approach to patient care.
• A valid driver’s license and reliable transportation.
