Responsibilities

• Providing hands-on nursing care including medication administration, treatments, and ADL support
• Working closely with the RN and team leads to support individualized care plans
• Completing documentation with accuracy and attention to detail
• Supporting Resident Assistants through mentorship, coaching, and positive leadership
• Managing medication orders, refills, and overall medication processes
• Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License

• Current Minnesota LPN license
• Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families

• Familiarity with Alzheimer's/dementia care is a plus
• Confidence using electronic documentation systems
