Job SummaryThe Licensed Practical Nurse (LPN) performs nursing care under the direction and supervision of the respective department Director/Manager and/or Shift Supervisor. The LPN oversees the work of CNAs on the unit and directs emergency response when necessary. The LPN assists providers (Physician, nurse practitioner, consultant pharmacist) on the unit as requested and, as the practicing licensed professional on the unit, ensures all documentation is timely and correct, all medications are dispensed as written, and that any/all errors are promptly reported and remediated.
EssentialJob Functions
- Supervises, assigns, and monitors work performed by CNAs; makes rounds to ensure quality nursing care is provided; provides backup support as needed and takes corrective action when necessary.
- Provides training to clinical staff; informs management of training needs; contributes to performance evaluations and discipline for nursing assistants.
- Complies with department and company policies and procedures including standards of conduct, attendance, dress, and safety.
- Assists with resolving staffing needs and emergency staffing problems when necessary.
- Maintains positive and professional relationships with residents, guests, and Williamsburg Landing employees; monitors staff for professionalism and positive interactions.
- Assists with planning and overall nursing care of residents on the unit; monitors medical records for accuracy and timely updates; assists with chart audits and 24-hour chart checks; documents tasks in electronic medical records.
- Performs professional nursing tasks including medication administration, ordering prescriptions, and treatments as ordered.
- Participates in shift handoffs with the previous and next shifts to ensure continuity of care.
- Keeps providers and clinical leaders informed of significant changes in resident conditions and signs of illness; reports concerns promptly.
- Assists management with resident evaluations to determine individualized care plans and attends resident review meetings as needed.
- Maintains communication with providers and family members as requested and within regulatory guidelines.
- Responds to resident needs including emergencies, medication administration, and assistance with activities of daily living.
- Addresses allegations or sightings of potential abuse/neglect by protecting and reassuring the resident and reporting the issue to management.
- Maintains access to and responsiveness of care base telephone or delegates as needed.
- Keeps work environment clean and tidy and ensures infection control protocols are followed.
- Maintains adequate supplies and equipment and records supplies to be ordered.
- Evaluates work processes and submits recommendations for continuous performance improvement.
- Responsible for own professional development, including meeting mandated standards and staying current with developments in the field; attends required trainings.
- Performs other duties as assigned and completes daily skilled notes to document resident observations and changes in condition, as well as delivery of medications and treatments; maintains residents’ medical records as necessary.
- Rounds on assigned residents at least twice per shift to ensure safety and adherence to the plan of care.
- In absence of the Director/Manager and/or Supervisor during weekend or evening/night shifts, responds to incidents or injuries to triage/provide first aid and investigates and documents events according to procedures; reports incidents promptly.
Must be a Licensed Practical Nurse in the Commonwealth of Virginia; ability to read and write effectively in English; two (2) years of nursing experience in long-term care, acute care, and/or assisted living setting preferred.
