About the Role

Ennoble Care is hiring for a Full-time Hospice Registered Nurse Case Manager to work within and around the Loudoun County region in Virginia. As a member of the interdisciplinary team, the RN Case Manager works under the general direction of the Director of Nursing, the attending physician, and the interdisciplinary group in providing skilled nursing assessments, planning, and care to maximize the comfort and health of patients and their families.

Responsibilities include identifying patient and family/caregiver needs and delivering supportive care in accordance with the attending physician’s orders and the plan of care.

  • Shall be assigned patient caseload by the Patient Care Coordinator or designee
  • Assume primary responsibility for a patient caseload that includes assessment, planning, implementation, and evaluation of the nursing process
  • Initiate communication with attending physicians, Hospice staff, and other agencies to coordinate optimal care and resources
  • Maintain regular communication with the attending physician regarding patient and family/caregiver needs
  • Seek input from team members regarding the patient plan of care to obtain additional knowledge and support
  • Provide accurate and timely documentation in electronic medical records
  • Participate in Interdisciplinary Group meetings and contribute appropriate information
  • Coordinate physical care of the patient by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate
  • Meet regularly with Hospice nursing staff to review caseload issues, share professional support, and exchange feedback
  • Attend patient care conferences and facilitate discussion of caseload issues for staff discussion and evaluation
  • Supervise and document the NAC every two weeks or as assigned
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues
  • Serve as associate nurse during off-hours or as requested to address patient/family needs
  • Provide appropriate support at time of death and during bereavement
  • Participate in Hospice orientation and ongoing training programs
  • Participate in agency and community programs to promote professional growth and understanding of hospice care
  • Maintain familiarity with agency policies and state/federal regulations governing hospice
  • Accept other assignments as appropriate
About You
  • Possession of a current license to practice as a registered nurse in the State of Virginia
  • At least two years of experience as a professional nurse, preferably with at least one year in hospice care
  • Ability to assess and respond to the needs of patients and families/caregivers in varied settings
  • Skilled in nursing practice, able to cope with emotional stress and support diverse lifestyles
  • Ability to make meaningful observations, conduct skilled assessments, and write comprehensive, accurate reports
  • Team player who supports an interdisciplinary approach to patient and family care
  • Valid driver’s license, good driving record, reliable vehicle, and ability to drive year-round
  • Ability to take direction, work independently, and collaborate effectively within a multidisciplinary team
