We are seeking a dedicated and compassionate nurse to join our dynamic healthcare team. In this role, you will play a pivotal part in building rapport and fostering relationships with health system staff while educating and identifying patients eligible for chronic care management services. Your expertise will help ensure that patients receive comprehensive support, leading to improved health outcomes and enhanced patient experiences.

ROLES & RESPONSIBILITIES:
• Relationship Building:
Develop and maintain strong relationships with health system staff, including physicians, nurses, office staff, social workers and administrative personnel, to facilitate effective communication and collaboration for Continuous Care referrals.
• Onsite Patient Engagement:
Provide education to patients and their families about available care coordination services, including the benefits of chronic care management and remote patient monitoring programs.
• Eligibility Identification:
Assess and identify patients who may benefit from care coordination services, utilizing clinical judgment and patient data to make informed recommendations.
• Care Coordination:
Work closely with the care coordination team to ensure seamless transitions of care for patients, including post-discharge follow-ups and ongoing support.
• Advocacy:
Serve as a patient advocate, addressing any concerns or barriers to accessing care and ensuring patients receive the support they need.
• Documentation:
Maintain accurate and up-to-date documentation of patient interactions, eligibility assessments, and care coordination efforts in compliance with health system policies.
• Training and Support:
Participate in training sessions and staff meetings to enhance team knowledge and foster a culture of continuous improvement in patient care services.
• Quality Improvement:
Collaborate with the quality improvement team to evaluate and enhance care coordination processes and outcomes.
• Care Delivery:
Provides in-person and telephonic clinical support for patients. Assess patient needs, plan and modify care and implement interventions. Provide education and coaching on remote patient monitoring (RPM) device set up and use. Establish and maintain effective working relationships with patients, providers, team members.

QUALIFICATIONS – EDUCATION, WORK EXPERIENCE,

CERTIFICATIONS:



Education:

Bachelor’s degree in Nursing (BSN) preferred;
Associate’s degree in Nursing (ADN) acceptable with relevant experience.
• Licensure:
Current Registered Nurse (RN) license in VA.


Experience:

Minimum of 5 years of nursing experience, preferably in care coordination, case management, or community health.

KNOWLEDGE, SKILLS, AND ABILITIES:

This position will require some travel between facilities.
Flexible hours may be required to accommodate patient needs and staff meetings.
