We are seeking a compassionate and dedicated Hospice RN Case Manager to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, ensuring comfort and support for patients and their families during end-of-life care. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provide direct nursing care, including medication administration, symptom management, and wound care.
  • Educate patients and families about the hospice process, available resources, and care options.
  • Coordinate care with other healthcare providers, ensuring continuity and quality of care.
  • Document patient progress and maintain accurate medical records in compliance with regulations.
  • Participate in team meetings and contribute to quality improvement initiatives.
Skills and

Qualifications:
  • Current Registered Nurse (RN) license in the state of practice.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of 2 years of nursing experience, with at least 1 year in hospice or palliative care.
  • Strong assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Compassionate and empathetic approach to patient care.
  • Knowledge of hospice regulations and standards of practice.

Requirements:
  • Current, unencumbered Virginia RN license.
  • One year of nursing experience, preferably in hospice, home health, or skilled nursing.
  • Current Basic Life Support (CPR) certification.
  • Proficient in English (spoken and written).
  • Excellent verbal and written communication skills.
  • Ability to meet deadlines and work independently.
  • Capable of performing CPR and airway obstruction relief.
  • Able to walk or stand for at least 4 hours and climb two flights of stairs.
  • Valid driver’s license with a satisfactory driving record and proof of auto insurance.
  • Must complete 12 hours of continuing education, annually.
