INSTITUTIONS & FORENSIC SERVICES MANAGER

SBH is seeking a qualified individual to be the responsible liaison between SBH and all state mental health facilities and private psychiatric hospitals for discharge planning, assessment of services needed, coordination of service delivery programs, and clear, ongoing communication with individuals served, the SBH discharge planning team, the facility treatment team, their families, and assigned therapists and case managers. This individual will represent SBH with effective, positive presentation skills and in a highly professional manner at all times.
The individual is required to write individualized service budgets, negotiate contracts with licensed, and private providers. The individual for this position also supervises the Forensics Coordinator and Forensics Case Manager to ensure all forensics cases are assigned and managed properly. This includes overseeing communications with the court system for the management of evaluations, restoration cases and management of NGRI both in the state hospital system and in the community.
Individual will also provide reporting on a regular basis on the use of funding and contracted services. May require work outside of normal office hours including evening and weekend hours.
- Serves as SBH’s liaison to all state hospitals to coordinate discharge planning with the hospital discharge planning team. May also be required to provide discharge planning for private hospitals, as needed.
- Follows state and regional guidelines and protocol for individuals in the state hospital system and has a thorough knowledge of the IDAPP manual and follows the manual in the creation of IDAPP plans.
- Works with discharge planning team, facility treatment, individuals, their families and all identified community providers to create a strength based, individualized discharge plan.
- Attends or coordinates treatment team meetings at state hospitals where individuals are identified at SBH clients.
- Finds appropriate placement for clients needing housing upon discharge in coordination with the hospital discharge planning team, Regional Utilization Management (RUM) Team and DBHDS
- Coordinates plans and maintains records for Discharge Assistance Program (DAP) funding for both civil and NGRI clients, and other funding streams that require reporting.
- Forming and maintaining relationships with local jails and other community legal partners including local law enforcement, parole and probation, Virginia Department of Corrections and court systems to deliver discharge and community release services.
- Any other duties assigned by Director of Adult Clinical Services in accordance with agency policies and procedures
- Master’s Degree in a Human Services field.
- 2 + years of experience in Community Mental Health, and/or Hospital Discharge Planning for adults is required.
- LPC, LCSW in VA required.
- Must meet all criminal background and drug screening requirements.
- Valid driver’s license and safe driving record, required.
