* Qualifications:

* Must successfully complete the required pre-employment Background and Drug Screenings
* Must possess a valid Driver's License which meets company standards for approved driver status
* Must possess basic computer skills
* Must possess a Bachelor’s Degree in the Human Services field
* Must have at least one year experience working in the field of intellectual/developmental disabilities
* Experience in writing program plans, case notes or other relevant documentation preferred

Essential Functions:

* Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires
* Creates positive, collaborative communication with all disciplines and team members
* Ensures that Program Plans and documentation are completed in accordance with all regulatory agencies
* Assists in monitoring and training of staff and relevant parties to ensure accurate, implementation and documentation of training, support and behavior programs as required by the individuals’ plans
* Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare
* Performs all other duties as assigned
