A health service organization is seeking a Case Manager to coordinate services for consumers to help minimize stress and assist in reducing psychiatric and substance abuse disorder symptoms. Responsibilities include offering support counseling, participating in a multi-disciplinary team, and providing crisis interventions. Qualifications include an Associate's degree in a related field and experience in Human Services. A valid driver's license and insured vehicle are required.
This role includes a $2,500 retention bonus.
