Home Care Physical Therapist

• Reports to the team coordinator
• Assumes responsibility and accountability for a designated group of patients and accurately provides Physical Therapy (PT) evaluations; develops and implements PT treatment plans; provides education and information to patients and caregivers; participates in required discharge planning and completes all required documentation according to policy.
• Supervises and assumes responsibility for provided Licensed Physical Therapy Assistants.
• May be assigned other duties such as clinical instructor for student affiliations, performance improvement, and program development.

  Degree in Physical Therapy from an American Physical Therapy Association accredited program
• Must have a current license as a Physical Therapist in the state of Virginia or be licensure eligible
• At least 1 year of post-graduate experience is absolutely mandatory
• BLS completion within 3 months of hire
• Experience working in a home care setting is strongly preferred but not required
• Ability to operate and apply therapeutic equipment; ability to lift/push/pull light to heavy equipment, supplies, and patients; creativity and problem solving skills
