Base Pay Range

$72,800.00/yr - $/yr

Job Requirements
  • Master’s Degree in Speech-Language Pathology from an accredited program.
  • Certificate of Clinical Competency or eligibility for Clinical Fellowship Year.
  • Current state licensure or license eligible.
  • Assumes responsibility for ongoing continuing education and professional development.
Physical Requirements
  • Exerting up to 20 pounds of force occasionally.
  • Exerting up to 10 pounds of force frequently.
  • Walking or standing to a significant degree.
  • Sitting while constantly pushing or pulling materials whose weight is negligible.
  • Stair climbing occasionally.
  • Overhead reach infrequently.
  • Average workday is up to 8 hours.
Job Responsibilities
  • Provides and directs speech therapy services to residents, including assessment, treatment, program planning & implementation, related documentation, and communication, functioning under physician’s orders.
  • Provides speech therapy services to residents and initiates referrals when appropriate and in accordance with Northwoods Rehab Policies and Procedures.
  • Conducts screenings of residents at regular intervals to determine need for intervention and/or treatment.
  • Evaluates residents to obtain data necessary for treatment planning & implementation; conducts specialized evaluations as indicated.
  • Interprets and communicates evaluation findings to residents, family, and other team members.
  • Establishes individual resident schedules in conjunction with other team members.
  • Develops treatment plans by identifying a target list, including short- & long-term goals & methods to achieve identified goals.
  • Coordinates treatment plan with resident, family and other team members.
  • Implements and conducts treatment as outlined in treatment plan.
  • Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
  • Develops appropriate home or programming within the community to maintain and enhance the resident’s performance in their own environment.
  • Documents results of resident’s assessment, treatment, follow-up & termination of services.
  • Adapts treatment to meet the individual needs of the resident.
  • Recommends & obtains appropriate equipment to improve resident’s communication; trains resident, family and appropriate staff in the use of augmentative communication devices.
  • Instructs resident, family and staff in appropriate speech-language procedures or home program to allow carryover of therapeutic remediation & goals in daily activities.
  • Participates in discharge planning.
  • Supervises Rehabilitation Techs/Aides, students and volunteers.
  • Reviews the quality & appropriateness of total services delivered and of individual speech therapy programs for effectiveness & efficiency, using predetermined criteria.
  • Participates in resident, family & staff education.
  • Confers formally & informally with other team members in coordinating total rehabilitation program of the resident.
  • Adheres to Northwoods Rehab procedures regarding documentation of speech therapy services.
  • Attends in-service meetings & community meetings as required.
  • Provides in-service education to the member(s) of the resident’s care team & to the community.
  • Provides treatment in accordance with established standards of practice & department procedures.
  • Maintains a safe & clean work area & adheres to community safety standards, plus all Infection Control, Universal Precautions & OSHA standards for the healthcare professional.
  • Consistently demonstrates sound judgment in evaluation, planning, implementation & follow-up of resident speech therapy programs.
  • Maintains positive relationships & rapport with coworkers, supervisors, residents, family members & community personnel & administration.
  • Assists with & participates in Quality Assurance Program as required.
  • Adheres to established confidentiality standards of the resident and organization.
  • Reports to work on time & coordinates schedule for efficient patient treatments.
  • Projects a positive & professional image at all times.
  • Maintains passwords for required software & keeps all passwords secure.
  • Performs other duties as assigned to effectively perform responsibilities of the position in best interest of Northwoods Rehab.
