Position Overview

Join a dedicated healthcare team providing patient care in post‑acute facilities within your community. The Advanced Practice Provider role involves managing and coordinating patient care, conducting rounds, writing orders, and collaborating with staff and physicians.

• Deliver exceptional care and communicate effectively with team members.
• Maintain accurate electronic medical records and documentation.
• Provide services aligned with policies and regulations.

• Excellent communication, empathy, organization, and teamwork.
• Preference for at least one year of relevant experience in geriatrics, internal medicine, ICU, emergency medicine, hospitalist, or general practice.
• Active Nurse Practitioner license and DEA certification.

The position is full‑time, day shift, with an on‑call schedule, offering $120,000-$135,000 annually. Applicants must be willing to travel to Lynchburg, VA. In‑person work is required.
