The Hoy CenterThe Hoy Center at Westminster-Canterbury on Chesapeake Bay takes a holistic approach to skilled nursing care and aging. Our person‑centric living aligns the delivery of care with individual needs and preferences. The Hoy features home‑like, comfortable settings, which we call Lighthouses, each with a family room and e‑concept kitchen. These modern and open private rooms are comfortable, well‑appointed and tastefully designed.
The Lighthouses include the Rose Island, Mission Point, Point Lookout, Cape Henry and Point Roberts.
Residents on each floor enjoy breathtaking views and refreshing ocean breezes on three spacious and striking outdoor terraces, we call Observation Decks. The Fletcher Prayer Room, which provides a dedicated, peaceful sanctuary for contemplation and prayer is also available to our residents.

What You'll Do In This Role- Perform blood glucose monitoring, inhalation treatments, and other physical prescribed medical treatments/diagnostic test.
- Maintain medication and resident's records, responsible for electronic input of resident's data in EHR system.
- Transmit requests including scans and/or faxes medication refill requests per policy.
- Communicate and coordinate care with residents and their families.
- Document and lead in the documentation of resident care.
- Inspect the environment, identify environmental problems and initiate requests for correction as needed.
- Identify, report and document incidents and accidents; participate in investigation as necessary.
- Review, transcribe and implement physician orders.
- Communicate with resident's physicians as directed.
- Implement, practice and assure staff adheres to infection control measures.
- Lead CNAs (Certified Nursing Assistant).
- Participate in continuing education programs to maintain competency.
- Support and participate in community and departmental quality assurance programs.
- Ensure that unit practices are consistent with regulatory requirements.
- Perform all other duties as assigned.
- Maintain positive working relationships between nursing staff and other departments.
- Compassionate, patient‑centered approach to care.
- Ability to work collaboratively within a multidisciplinary team.
- Must be able to climb or descent 3 flights of stairs.
- Must be able to stand or walk for an extended period of time and push a wheeled medication cart through assigned unit.
- Active, unencumbered LPN license in the state of Virginia (or compact)
- Current CPR certification
- Must have at least one year of nursing experience, one of which is in a long‑term care facility.
- Flu‑Vaccinated
- Experience with Microsoft and Google Applications
