Hospice Nurse Practitioner; part-time

Overview

Make a difference daily as a Hospice Nurse Practitioner! This role involves providing compassionate, patient-centered hospice care in various settings, including home and facilities. Responsibilities include diagnosis, treatment, medication management, and collaborating with an interdisciplinary team to ensure quality care and support patient and family goals.

• Conducting face-to-face visits and care coordination
• Prescribing medications in collaboration with physicians
• Participating in on-call rotations for 24/7 coverage
• Engaging in community outreach and education
• Maintaining accurate documentation and adhering to quality standards

Qualifications include licensure, certification, and experience in hospice or palliative care. Essential skills involve strong communication, clinical expertise, teamwork, and patient advocacy. The role requires physical ability to work in various environments and participate in on-call duties.
