Role Snapshot

• Role: Licensed Practical Nurse (Home Health)
• Location: Virginia Beach & Norfolk, VA
  • Onsite (home visits, local travel only)
• Pay: $48 per visit (~$70,000/year avg.)
• Schedule: Monday–Friday, 8am–5pm
  • 5–6 visits/day
• Work with a stable, Medicare-certified home health team serving Hampton Roads. Clear routes and consistent workload.

• Conduct home health visits and basic assessments
• Deliver wound care, med administration, skilled nursing
• Support start-of-care and follow-up visits (no OASIS lead)
• Coordinate with RN case managers and therapists
• Complete visit notes within 72 hours
• Communicate changes in patient condition promptly

• Active LPN license in Virginia (good standing)
• 6+ months of home health experience
• Current CPR certification
• Valid driver’s license and reliable insured vehicle
• Must pass OIG, state, and national background checks

• Pay: $48 per visit (~$70K/year based on average caseload)
• Health, dental, vision
• PTO + paid holidays
