The Role:
Move-On Coordinator, Hampton Roads, VA
In this grant-funded position, you ll work across multiple supportive housing communities to conduct assessments and assist residents with their individualized move-on goals. The successful incumbent has experience initiating and sustaining a high level of accountability resulting in positive outcomes, is mission-focused, and demonstrates previous experience successfully collaborating with all levels of staff, volunteers, community, and board members.
- Provide daily oversight of the move on program to include, but not limited to, case management guidance related to assessing current needs, establishing move on plans, linking to housing vouchers, identifying housing needs, assisting with moving, connecting with needed resources in the community, and providing follow up services to support housing stability and track progress during the first transition year
- Provide case management expertise to include, but not limited to, assessments, treatment planning, supportive counseling, housing stabilization services, case management services, linking to community resources, monitoring progress, collaboration with other providers, and care coordination
- Provide the necessary assessment, case management services, coordination of resources, follow-throughs, and documentation to assist a minimum of 50 residents annually with move-on interventions
- Organize, facilitate, and/or coordinate appropriate life skills training, workforce development, job training, vocational services, tenancy skills, and financial literacy to promote self-sustainment and employability skills
- Adhere to the grant requirements and assist with the annual grant reporting documents
- Ensure compliance with all required reporting, data entry and quality control in accordance with agency, HUD, CoC, and HMIS standards
- Assist with necessary outreach and engagement activities to include residents, their supports, community providers, housing authorities, and property management
- Master s degree in human services with a minimum of three years of work experience supervising and providing direct services to individuals with low-income, physical disabilities, behavioral health and developmental disorders, and/or history of homelessness OR Bachelor s degree in human services with a minimum of five years of work experience as indicated above
- Prior experience in a human services environment supported by grants required
- Knowledge of electronic health records/data management preferred
- Knowledge of homeless population and appropriate community resources, especially entitlement and housing resources
- SOAR certification and experience strongly preferred
- Knowledge of mental illness, substance abuse disorders, community resources, medication management, and recovery concepts
- Certification in CPR and First Aid required or obtained upon hire
- Excellent communication skills including verbal, written, presentation and listening plus excellent Microsoft Office computer skills
- Valid VA driver s license and clean driving record for local/state wide travel as needed

Salary: $28.36 - $29.32 per hour
