The LPN assigned to this clinic performs assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran.

The Community Based Outpatient Clinic (CBOC) PACT LPN provides skilled and individualized practical nursing care to all levels of patients, including the most complex patients who are acutely and/or chronically ill in the outpatient care setting. The incumbent serves as a member of a health care team / teamlet in Primary Care. The LPN demonstrates active collaboration and communication with the Core Team (Veteran, Primary Care Provider, RN Care Manager, and Clerical Associate) and expanded team (family/caregiver, internal and community-based services involved in providing care to the patient).
