TITLE Health Connection Manager
REPORTS TO Chief Executive Officer
POSITION CLASSIFICATION Regular Full-Time, Exempt
SALARY RANGE $48,185 - $60,558/ annually
Responsible for the daily execution, development, implementation, and management of Health Connection service area. Incumbent will ensure the proper accountability of programs consistent with funding source guidelines, agency policies and procedures.
Skills & Education RequirementsDegree in related field and/or equivalent knowledge and experience. Previous management / supervisory experience preferred.
- Effectively communicate both orally and in writing; have the ability to write reports and business correspondence; have the ability to effectively present information;
- Cooperate successfully as a member of a team;
- Possess good organizational skills, with sensitivity to details; and
- Proficiency with computer operations and possess skills preferably with MS Office
Job Requirements
- Must possess a valid driver’s license and have access to a personal vehicle with required liability insurance for use in business related travel
- Must ensure efficient operations and present a professional image in conduct, attitude and attire
- Must be philosophically compatible with the mission of PACE

- Acquire an in-depth knowledge of all programs and outside resources available to meet the needs of low-income clients. Collaborate with internal programs and use other resources to assure maximum benefit to families
- Oversee and monitor the Health Connection program ensuring compliance with funding source requirements, including adherence to contracts, budget allowances, and expenditures
- Conduct program reviews and analyze results to determine programmatic updates needed
- Oversee and coordinate the 340B program process, vendor contracts, and program meetings
- Set priorities for yearly program development, expansion, or reduction.
- Participate in the maintenance of quality care standards in the clinics
- Recommend staffing patterns including hires, promotions, transfers, and terminations
- Supervise and evaluate assigned staff, implementing Professional Development Plans and providing ongoing training
- Ensure a work-safe environment for all staff
- Provides oversight for multiple locations, including regular travel to each site
- Attend all assigned training and meetings.
- Submit timely, accurate and complete reports
- Comply with company, state, and federal regulations
- Participate as part of the administrative team being a liaison between staff and administration
- Provide on-going communication and information sharing with staff, participants, and community members
Requires sitting, standing, stooping, bending and reaching with manual dexterity sufficient to operate standard office machines such as computers, fax machines, calculators, copy machines, telephone and other office equipment. May require lifting up to 40 pounds. Requires normal range of hearing and vision. Must be able to handle diverse work problems on a daily basis. Requires ability to set priorities and work schedule, yet adjust to changes necessitated by last minute assignments and deadline requests that may prove stressful.
Consistent pleasant attitude is necessary with personal maturity as an important attribute. Must relate and interact with people at all levels of the company and in a culturally diverse environment.
