Overview
Good Samaritan Hospice Indiana - Illinois is hiring a full-time Medical Social Worker. The role offers a standard 5-day work week (8:00 AM – 5:00 PM) with no weekend or after-hours on-call requirements. New pay rates and competitive benefits are included, along with mileage reimbursement and tuition reimbursement.
Responsibilities- Establishing a care plan in accordance with the physician’s plan of treatment.
- Providing crisis intervention as required.
- Assisting the physician and other caregivers in understanding the social, emotional, and economic factors influencing the patient’s medically related care.
- Providing patient care education to patients and caregivers.
- Assisting in making appropriate community referrals to gain access to additional resources (e.g., applications for public assistance, housing problems, additional health care resources, transportation).
- Collaborating with the patient’s caregivers to resolve problems that affect the caregiving relationship and care outcomes.
- Completing and submitting all required documentation accurately and timely.

- Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
- Currently licensed to practice in the state served.
- One year of social work experience in a health care setting.
- Current CPR/BLS certification through the American Heart Association or American Red Cross and compliance with state requirements.

Pay Range$25.54 - $38.00 per hour
