SummaryWe are seeking a Licensed Clinical Social Worker (LCSW) to join our interdisciplinary pain management team. This role is vital in supporting patients’ emotional, psychological, and social well-being as part of a structured chronic pain treatment program.
Candidates with experience in pain management, hospice, or palliative care are strongly encouraged to apply. This is an outpatient role with predictable hours, no weekends, and no on-call responsibilities.
Responsibilities- Conduct individual clinical sessions addressing chronic pain and mental health needs.
- Provide evidence-based therapy including CBT, ACT, and mindfulness-based approaches.
- Monitor patients for changes in mood, cognition, or behavior related to medication therapy.
- Conduct psychosocial assessments including trauma history and social determinants of health.
- Provide crisis intervention and emotional support.
- Collaborate with physicians and clinical staff to coordinate treatment plans.
- Serve as liaison between patients, families, and the care team.
- Assist with insurance navigation and community resources.
- Maintain accurate and timely EMR documentation in compliance with HIPAA.
- Master’s degree in Social Work (MSW) required.
- Active Illinois LCSW license required.
- Minimum of 2 years of clinical experience preferred.
- EMR documentation proficiency (eClinical
Works). - Strong communication and teamwork skills.
- Hospice or palliative care experience.
- Experience in psychotherapy and crisis intervention.
- Experience in chronic pain or integrated medical settings.
- Experience monitoring patients on complex medication regimens.
- Training or experience in CBT, ACT, or mindfulness-based therapy.
- Experience working within an interdisciplinary healthcare team.
- Monday through Friday, 9:00 AM to 5:00 PM
