The Registered Nurse (RN) in Home Health provides skilled nursing care, comprehensive assessments, care coordination, and clinical oversight. The RN completes OASIS assessments within required time frames, collaborates with interdisciplinary team members, and ensure high-quality, patient center care. The RN may serve as Alternate Disaster Coordinator if assigned.
JOB DUTIES AND RESPONSIBILITIES- Performs comprehensive assessments of patients at start of care, resumption of care transfer, recertification, and discharge, including accurate evaluation and completion of all OASIS data. Documentation must be completed and in “submitted with signature” status within required agency time frames. (EF)
- Performs skilled nursing visits, including supervisory and hightech visits such as wound vac management, tracheostomy care, gastrostomy care, central line care, and other complex procedures. Ensures documentation is completed and submitted within required agency time frames. (EF)
- Can act as Alternate Agency Disaster Coordinator in the absence of the Administrator or Alternate. (EF)
- Delegates appropriate tasks to LVNs and Home Health Aides and provides supervision in accordance with state regulations to ensure safe, effective, and compliant patient care.(EF)
- Provides patient care in accordance with the physicianordered plan of care and documents interventions accurately and completely in accordance with federal, state, and agency standards.(EF)
- Participates in the agency’s OnCall rotation, including one weekday per week, rotating weekends, and designated holidays, triaging calls, completing PRN or urgent visits as needed, and coordinating appropriate follow-up. (EF)
- Completes and submits infection control reports, incident/occurrence reports, and other required documentation promptly and accurately.(EF)
- Participates in clinical record audits and reviews to ensure completeness, accuracy, and compliance with regulatory requirements.(EF)
- Participates in the PI and QAPI program. (EF)
- Assist the RN Case Managers when needed. (EF)
- Communicates and collaborates with members of the interdisciplinary team—including physicians, therapists, aides, and office staff to ensure coordinated, patient centered care.
- Maintains compliance with all federal and state Home Health regulations, agency policies, and professional standards of nursing practice.(EF)
- Adheres to organization-wide and other applicable policies and procedures.
- Day-to-day performance complies with the hospital’s Service Excellence Behavioral Expectations.
- Performs within the prescribed limits of the hospital’s Ethic, Compliance and Confidentiality Program guidelines.
- Performs within professional Scope of Practice at all times.

Required:
Graduate of an accredited college, university, or school of nursing
Required:
6 months in a hospital setting
Preferred: 1 year in a hospital setting and previous home health experience
SpecialSkills and Abilities
Required:
Strong computer, communication, written, time-management, and organization skills. Ability to perform effectively in stressful or rapidly changing situations.
Preferred:
Wound care skills, central line skills, Oasis knowledge, Well Sky EMR
Required:
Current License Registered Nurse by the Texas Board of Nursing, Valid Driver’s License, auto insurance, reliable transportation, and current CPR certification.
