RN - Case Management

Shift: 8:00 AM - 4:30 PM with rotating weekend shifts.

The Travel Care Manager RN will be responsible for overseeing the care planning and discharge process within the hospital setting. The role involves ensuring effective utilization review (UR) practices are followed to promote quality patient care. The ideal candidate will have recent experience in discharge planning and a strong background in hospital case management.

• Manage and coordinate patient discharge planning and transitions of care.
• Perform utilization reviews to ensure appropriate resource management.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Document case management activities using Mor Care and assist with referrals via Care Port.

• BLS certification is mandatory.
• Must have recent experience in discharge planning and UR in a hospital.
• Minimum of 3 years of Acute Hospital Case Management experience is required, with preference given to candidates with travel experience.
• Must have a permanent address more than 50 miles from the facility.
