Position: Referral Coordinator | New Full-Time Opportunity!

BASE PAY RANGE

$45,000.00/yr - $50,000.00/yr

SCHEDULE

Monday-Friday 8am-4pm

JOB SUMMARY

The Referral Coordinator plays a pivotal role in facilitating the coordination of client referrals across Hope Clinical Care. Key responsibilities include acting as a liaison between patients and the clinical teams and billing teams, providing high quality customer service, and aligning performance with the strategic goals of the organization.

ESSENTIAL FUNCTIONS
  • Manages client intake processes, which includes, but is not limited to fielding incoming requests for services, being the first point of contact for potential families/caregivers, responding to all intake communication and correspondence (phone, email, online application, and fax); and responding to service inquiries in the Signify portal for the TAP program.

    • Works closely with HCC program leadership to manage and understand waitlists and clinician utilization
    • Quickly selects qualified clients for onboarding
    • Facilitates a speedy onboarding process and champions collaboration with the clinical and billing teams
    • As needed, schedules clinical appointments for evaluations and ongoing therapy and diagnostics
  • Provides each potential family/caregiver and existing family/caregiver with a high touch experience, accurately describing the process for client intake onboarding, answering questions, and providing useful resources. Works quickly and diligently to support potential customers as part of the intake processes.

  • Coordinates with medical professionals to obtain required documentation, like reports and prescriptions for services.

  • Manages all client onboarding documents including billing registration forms, ROIs, consents, and other intake paperwork, ensuring that it is housed in the client’s electronic medical record, focusing on HIPAA compliance.

  • Maintains accurate documentation associated with the client pipelines and onboarding steps, analyzes data, and provides accurate metrics for speed to service.

  • MARGINAL FUNCTIONS
    • Attends required meetings
    • Completes other tasks, as requested
    • Acts as a mandated reporter
    • Cross functional collaboration with other Hope departments
    WORKING CONDITIONS

    The position functions are clinic-based, requiring the employee to be on-site; work is primarily performed in an indoor office environment. Works very near others, within a few feet or less; involves close contact with clients and BBP; exposure to moderately loud sounds and high noise levels intermittently; may involve conflict situations with parents or public constituents.

    PHYSICAL REQUIREMENTS

    The Referral Coordinator’s role is subject to prolonged periods of sit-down work at a desk and working on a computer; must be able to lift, push, pull and carry a minimum of 25 pounds at a time. This position may require the ability to bend, twist or stoop on a repetitive or continuous basis, operate various business instruments including computers, iPads, copiers or fax machines requiring manual dexterity.

    Document analysis and organization may require extended levels of concentration as well as visual acuity to read paperwork and computer monitors.

    • Skilled in problem solving and flexibility in difficult situations.
    • Proficient at Microsoft Excel and Microsoft Office Suite, telephone, and fax
    TRAINING, ACADEMIC AND EXPERIENCE REQUISITES

    Qualified candidates will have an associate degree in psychology, sociology or other human services related field, with at least two (2) years working in a human service setting.

    PREFERRED QUALIFICATIONS

    Preferred candidates possess a bachelor’s degree with experience in a clinical or therapeutic professional setting with experience in customer service.
