JOB DESCRIPTION
Under direct supervision of the Care Management and Life Enrichment Director the Social Worker provides social services to Woodridge residents and their families to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident while adhering to state and CMS guidelines. Responsible for patient assessments, care planning and helping patients and families adjust to their new surroundings.
EDUCATIONMinimum of a Bachelor’s degree in a health related field including but not limited to sociology, counseling, human services, community wellness and psychology. One year of experience in a health care setting working directly with individuals. Must demonstrate knowledge of the issues and developmental stages associated with geriatric residents. Must be able to assess data related to age-specific needs and provide care as described in the department’s policies and procedures.
EXPERIENCEA minimum of one year’s social work experience in a health care setting.
