As an SLP, you will evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their educational environment. In addition, this position is responsible for clinical supervision of students and SLPs in CFY year and students. Responsibilities include but are not limited to the following:
- Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to calling patients to reschedule visits, reshuffling patient load to meet new demands, and communicating recommended changes to supervisor.
- Performs a comprehensive (OASIS and SLP) assessment of assigned patients, including psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
- Develops a plan of care to promote optimal functional independence and safety as documented by the clinical record.
- Plans for and provides skilled speech-language therapy treatment, including therapeutic techniques and patient/caregiver education, assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
- As Care Manager and/or team member, coordinates patient care with other clinical team members, including physicians, nurses, caregivers, other rehab services and students, ensuring optimal patient care and communication in the clinical record.
- Accurately and completely performs functions related to school-based services, including evaluations, documentation, development of Individual Educational Plans (IEPs), participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork.
- As necessary, assesses patients' needs for Home Care Services, develops and coordinates the Plan of Care (POC) with patient/caregiver, and provides ongoing supervision.
- Coordinates safe patient discharges and/or transfers and ensures all appropriate paperwork and patient information are processed.
- Serve as a Home Care Speech-Language Pathologist as required.
- Master's degree in Speech Language Pathology required
- Current Vermont State SLP license required
- Two years of clinical Speech Language Pathology experience required
- Active enrollment in clinical fellowship year or successful completion of the Certificate of Clinical Competency required
- Valid Driver's License
