Location: Windsor

Join to apply for the Case Manager - Windsor role at Senior Solutions Vermont (Council on Aging for southeastern Vermont).

Base pay range: $22.00/hr - $24.00/hr.

We are a 2025 VT Best Places to Work and seek a dedicated, caring individual to join our growing case management team in Windsor, VT.

Qualifications
  • Bachelor’s degree in human services preferred; a combination of relevant education and experience may substitute for a degree.
  • Prior case management experience preferred.
  • Strong organizational, communication, and computer skills.
  • Reliable transportation and a valid driver's license.
  • Passion for working with seniors or adults with disabilities.
Responsibilities
  • Collaborate with community partners to create a person-centered plan of care.
  • Monitor and reassess program service plans as required.
  • Maintain working knowledge of services and programs available to older Vermonters and people with disabilities.
  • Ensure that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
