Cedar Hill Continuing Care Community, a family-owned and operated senior living community in Windsor, VT, offers a supportive home for seniors at all stages of life.

This is a full-time, on-site position for a Nurse at Cedar Hill Continuing Care Community, Skilled Nursing facility located in Windsor, VT. The Nurse will provide direct patient care, monitor resident health, administer medications, wound care,and follow care plans for long term and rehabilitation residents. Additional responsibilities include collaborating with healthcare teams, educating residents and their families, and maintaining accurate patient records.

The role is integral in ensuring a comfortable, safe, and supportive home for residents.

• Proficiency in patient care, including medication administration, wound care, and monitoring vital signs.
• Strong assessment, decision-making, and care planning skills to address residents' health needs effectively.
• Knowledge of CMS guidlines, safety standards, and infection control procedures.
• Effective verbal and written communication skills to collaborate with healthcare teams, residents, and families.
• Compassionate, empathetic, and resident-centered approach to caregiving.
• Current nursing license (LPN or RN) and compliance with required certifications.
• Experience in geriatric care or long-term care settings is preferred.
• Organizational skills and the ability to multitask in a dynamic environment.
• CPR certification and commitment to ongoing professional development are beneficial.
